Vagus Nerve and Inflammation

The vagus nerve regulates inflammation through what researchers call the cholinergic anti-inflammatory pathway. It senses inflammatory signals in the body, reports them to the brain, and sends back a message that tells immune cells in the spleen to ease off their production of inflammatory chemicals. Higher vagal tone means a stronger, faster version of that brake. Low vagal tone means the fire keeps burning after the threat is gone.
Your nervous system has a say in your immune system
For most of the last century, the nervous system and the immune system were taught as separate departments. Nerves did sensation and movement. Immunity did infection and repair. They were not thought to talk.
That turned out to be wrong. The vagus nerve, which wanders from your brainstem down through your throat, heart, lungs, and gut, is in constant conversation with your immune system in both directions. Roughly eighty percent of its fibers are sensory, carrying information up from the body to the brain. Among the things those fibers report is inflammation itself. Your brain is being told, in real time, how much fire is burning and where.
What comes back down is the part that matters here.
The inflammatory reflex, in plain English
Think of it as a thermostat with three parts.
- The sensor. Vagal sensory fibers detect inflammatory messengers such as interleukin-1 and TNF in the tissues and relay that signal up to the brainstem.
- The circuit. The brainstem processes the message and sends a signal back down the vagus nerve, which routes through a nerve connection to the spleen, the body's immune hub.
- The brake. There, acetylcholine, the vagus nerve's chemical messenger, meets receptors on immune cells called macrophages and quiets their production of inflammatory cytokines, particularly TNF.
The whole loop is fast, automatic, and running right now without your permission. Kevin Tracey and colleagues mapped it in the late 1990s and named it the inflammatory reflex, and it reframed inflammation as something the nervous system actively governs rather than something the immune system does on its own.
Inflammation, understood this way, is not the enemy. It is a fire your body lights on purpose to fight infection and repair damage. The problem is never the fire. It is a fire nobody puts out.
What happens when vagal tone is low
If the brake is weak, the fire runs long. That is the state we now recognize as chronic low-grade inflammation, and low vagal tone tends to travel alongside it. Researchers repeatedly find an inverse relationship between heart rate variability, which reflects vagal activity, and inflammatory markers like C-reactive protein and IL-6. Lower HRV, higher inflammation.
Chronic stress makes this worse in a very ordinary way. Stress keeps the sympathetic branch elevated, vagal brake pressure drops, and the anti-inflammatory signal gets quieter at exactly the moment the body needs it most. This is one of the mechanisms behind the tired, achy, foggy, slow-to-recover state so many people live in and cannot get a diagnosis for. What the evidence actually supports
I want to be precise here, because this is a topic where the internet gets loud fast.
The strongest human evidence comes from implanted vagus nerve stimulation. In small trials, patients with rheumatoid arthritis who received an implanted stimulator showed reduced TNF production and improved disease activity scores. That is real, and it is remarkable, but it is a surgical device delivering a precise electrical dose.
Non-invasive approaches, including transcutaneous stimulation at the ear and slow breathing practices, show more modest and less consistent effects on inflammatory markers. Slow-paced breathing has been shown to raise vagal activity reliably. Its downstream effect on inflammation is promising and biologically coherent, but the human literature is younger and the effect sizes are smaller than the headlines suggest.
So the honest position is this: the mechanism is well established, the surgical evidence is strong, and the everyday-practice evidence is early but pointed in the same direction. Breathing daily will not treat an autoimmune disease. It will raise vagal tone, and vagal tone is the dial that governs the brake. Those are two different claims, and I will not blur them for you.
How to train the brake
Everything that raises vagal tone strengthens the pathway. The tools are not exotic.
- Slow breathing with long exhales. Five to ten minutes daily, in through the nose for four, out for six or eight. The exhale is where the vagal brake engages.
- Heat. Regular sauna use is associated with lower inflammatory markers and improved cardiovascular outcomes in long-running Finnish population research. It also leaves you in a deep parasympathetic state afterward. More on that in sauna benefits for your nervous system.
- Cold exposure. Cold triggers the dive reflex and a strong vagal rebound, and repeated exposure trains a faster recovery from stress generally.

- Sleep and movement. Unfashionable and non-negotiable. Poor sleep raises inflammatory markers on its own, and no practice compensates for chronic sleep debt.

Why the ancients kept putting the body through things
Every tradition I have studied built deliberate hardship into its practice. Sweat lodges. Ritual cold immersion. Fasting. Long chant. These were not tests of toughness, whatever we have turned them into. They were structured stress with a structured return, over and over, until returning became who you were.
That is what contrast is doing physiologically. Heat and cold in sequence, choosing a hard thing on purpose so the ones you did not choose cannot break you. Your body gets fluent in the round trip: threat, response, recovery, calm. And the anti-inflammatory brake is part of the recovery half of that trip. Train the return and you train the brake.
